With a highly skilled and dedicated renal team, we ensure ultimate safety and optimal treatment outcomes for our ESRD patients in the comfort of their homes using the FDA-approved NxStage System One technology.
Our services primarily include 24/7 clinical and technical access with medication delivery to the patient’s home, weekly physician visits, and online access to full dialysis medical records along with ambulance facilities and monthly laboratory logistics.

PRIMARY JOB FUNCTION
Responsible for the case management of assigned patients in all regions in UAE, in accordance with the agency policies and all applicable laws and relevant professional standards.
CORE FUNCTIONS/RESPONSIBILITIES
- Performs comprehensive subjective and objective assessment of the patient status that includes physical, psychosocial, and environmental parameters.
- Ensures compliance with regulations in DOH and DHA governing home health care services by following policy and procedure consistent with agency standards.
- Formulates an individualized plan of care that incorporates analysis of assessment data.
- Provides skilled interventions aimed at achieving realistic outcomes within a specified time.
- Modifies and updates plan of care to reflect progress toward outcomes.
- Assures thorough and accurate documentation with all clinical data including but not limited to; Admission through discharge, physician’s orders, skilled nursing visits, and changes in conditions.
- Consistently demonstrates competency with technical nursing skills according to the legal scope of practice.
- Maintains data collection, recording, and reporting systems to ensure proper service, data collection, and measurement of outcomes.
- Implements teaching specific to patient and family needs and evaluate and modifies as needed. 10. May participate in the Professional Advisory Committee.
- Participates in in-service education programs.
- May assist with quarterly clinical record reviews.
- Appropriately documents all applicable data in the patient’s medical record.
- Ensures the maintenance and confidentiality of patient records
- Arrange and participate in the initial assessment.
- Submits documentation timely and per HHD agency policy.
- Available for on-call as designated by HHD Management.
- Develops and turns in weekly schedule prior to the next week.
- Maintains productivity standards as established by the corporate office.
- Allows time in the schedule to do work from the HHD office.
- Plans direct and participates in the delivery of professional services to home care patients.
- Identifies patients who are appropriate for care conferences.
- Attends staff meetings.
- Focuses services on diagnosis-specific patient care needs.
- Seeks consultant expertise with the Administrator as needed to ensure agency growth.
- Agency Management approves time off, which is presented in advance in writing.
MARKETING
- Is willing to participate in the marketing efforts by acquiring patients in all UAE regions
- Participates in community relations activities as directed by HHD Management.
- Promotes the agency through positive representation and communication of its services.
- Participates in committees and community relations.
- Coordinate with the physicians in government hospitals and private hospitals to get more patients.
QUALIFICATIONS & REQUIREMENTS
- Graduate of an accredited nursing program with a current license DHA or DOH
- Knowledge and ability to provide skilled nursing care as defined in the nurse practice act.
- Current CPR certification.
- Minimum One (1) Year of home care experience.
- Knowledge and ability and the necessary skill and judgment to perform all nursing procedures as outlined in the scope of practice in the Nurse Practice Act.
- Should have a valid driver’s UAE license and use of an insured auto or access to adequate transportation.
- Minimum of two (2) years experience preferred in supervisory or administrative positions.
- Excellent verbal and written communication skills, and strong interpersonal skills
- Fluency in Arabic and English language
